I think the forum is the right place for the census. There's never doubt about the availability of census data - if the forum is up, the census is up too. We don't usually do anything "interesting" with the census data other than maybe counting the number of active citizens, and that's easy enough to do on the forum, so there would hardly be a benefit in getting the answers in a cleaner format.
One thing that I do think we should do is pare down the form a bit. Moving the "optionals" line three items up would be a good start.
